Position Responsibilities

Responsibilities include but are not limited to:

✔️ Providing skilled nursing care and prescribed treatments
✔️ Assessing patient conditions and implementing nursing care plans
✔️ Monitoring and evaluating patient progress
✔️ Coordinating care with physicians, therapists, nutritionists, and social workers
✔️ Supervising LPNs, CNAs, HHAs, and subordinate staff as needed
✔️ Identifying physical and emotional health concerns through assessment and observation
✔️ Maintaining accurate nursing documentation and records
✔️ Supporting patients and families with education and guidance

Qualifications

✅ Active New York State Registered Nurse (RN) License REQUIRED
✅ Previous nursing home or long-term care experience preferred
✅ Strong clinical judgment and communication skills
✅ Ability to work independently and collaboratively within a healthcare team
✅ Compassionate and patient-focused approach to care
