Schedule

🕒 Evening Shift: 2:45 PM – 11:15 PM

📍 Facility Location:
Willow Point Rehabilitation & Nursing Center

Position Responsibilities

Responsibilities include but are not limited to:

✔️ Providing professional nursing care and treatment to residents

✔️ Administering medications and therapeutic treatments (excluding IV administration)

✔️ Monitoring resident conditions and reporting changes to the Charge Nurse

✔️ Assisting with development, implementation, and evaluation of resident care plans

✔️ Maintaining accurate nursing documentation and medical records

✔️ Supervising Nursing Assistants and unit support staff as needed

✔️ Observing and documenting resident progress and responses to treatment

✔️ Securing and accounting for controlled medications

✔️ Participating in staff meetings and continuing education programs


Qualifications

✅ Active New York State LPN License REQUIRED

✅ New York State Limited Permit accepted (must obtain full licensure within 6 months)

✅ Previous nursing home, rehabilitation, or long-term care experience preferred

✅ Strong communication and documentation skills

✅ Ability to work effectively as part of an interdisciplinary healthcare team

✅ Dependable, professional, and resident-focused
